Issue:

Cannot create unique index; duplicate keys found Create Unique Index FAVORITE_FOLDERS_PATH_IX on Favorite_Folders(FF_PATH,FF_OWNER,FF_MODULE)

Solution:

There were duplicate entries in the Favorite Folders table. Make sure the PATH column does not have duplicate values. For example, we had AAAA and AAAB duplicated 6 times so we deleted each duplicate value great than ID 10
